Challenge
The Pharmacy Unit of the Barcelona Health Consortium (CSB) organized a meeting to analyse the collaboration between Primary Care and Pharmacy Offices.
The meeting was attended by representatives of the pharmacists and primary care physicians from different healthcare providers and the coordinators of the pharmacy offices in the city of Barcelona, in order to begin a joint work in this area. The purpose of the meeting was to find out the specific needs detected between Catalan Health Service, the Pharmacy Offices and the Primary Health Care Teams, and the result was the formal constitution of the working group of Primary Care and Pharmacy Offices from Barcelona city.
Once the needs have been identified, the challenge is to define consensus among all the stakeholders which will be the first steps.
Answer
A participatory day is held with the aim of defining actions to improve coordination and shared work (communication, training, participation) between primary care (AP) and the pharmacy (OF) offices in Barcelona city, in order to of improving care for patients and ensuring proper use of medication (effective, safe and efficient).
The result of this conference is a report that presents 10 proposals for improvement in communication, training and participation by consensus.
